As a courtesy to our patients with dental insurance coverage, we are happy to file your claim on your behalf to your insurance carrier. We ask that you become familiar with your program benefits and financial obligations. Our staff is here to help estimate your benefits. It is important to remember that the insurance contract is between you and your insurance company.

When calling to inquire about your dental benefits please have the following information available:

• the insurance company’s name, phone number and mailing address
• the policy holder’s name, date of birth and social security number
• the patients name, date of birth and social security number
• the group number and member id

 Your dental plan is designed to share your dental care costs. The total cost of your bill may not be covered by your insurance plan. The percentage of cost insurance companies pay varies drastically. Please check with your insurance plan for details.
